We are seeking a highly skilled Mental Health Coordinator to join our team at Wexford Health. The successful candidate will be responsible for delivering high-quality mental health services to our patients in the Western Virginia Regional Jail.
Key Responsibilities- Assess and screen new arrivals to determine mental health needs and develop individualized treatment plans.
- Coordinate care with the mental health team, community providers, and community supervision agencies to ensure continuity of care and stabilization of mental health conditions.
- Prepare patient lists for telepsychiatry clinics and coordinate patient attendance with security staff.
- Review patient assessment documentation and records with psychiatrists to ensure accurate and effective treatment.
- Prepare fee slips for clinic visits and medication orders, and collect patient signatures.
- Ensure confidentiality of medical records and patient care information, and validate and release information as directed by authorized personnel.
- Assist in preparing periodic statistical reports and other management information system requirements.
- Licensure: LMSW, LCSW, LPC, LCPC
- Certification: Current CPR certification
- Education: Master's degree in Counseling or Social Work
- Preferred Experience: At least one year of prior clinical experience working with forensic clients, familiarity with relevant research literature, clinical assessments, procedures, and methods.
