Job Summary

We are seeking a highly skilled Clinical Documentation Specialist to join our team at Ensemble Health Partners. As a Clinical Documentation Specialist, you will play a critical role in ensuring the accuracy and completeness of patient medical records.

Key Responsibilities
  • Conduct thorough reviews of patient medical records to identify and correct any discrepancies or inaccuracies.
  • Collaborate with physicians and other healthcare professionals to obtain necessary documentation and clarify any unclear or missing information.
  • Assign and validate ICD-10-CM and PCS codes, as well as DRG assignments, to ensure accurate reimbursement and coding.
  • Develop and implement strategies to improve clinical documentation practices and reduce denials.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Maintain accurate and up-to-date records of patient information, including medical history, diagnoses, and treatments.
  • Communicate effectively with healthcare teams to ensure seamless care coordination and patient flow.
Requirements
  • Minimum of five years of acute care nursing experience, with specific medical/surgical, Intensive Care, or Emergency Department experience.
  • Excellent interpersonal and communication skills, with the ability to work effectively with physicians, nurses, and other healthcare professionals.
  • Ability to analyze complex medical information and make sound judgments.
  • Proficiency in electronic medical records systems and coding software.
  • Current RN licensure and CRCR certification within nine months of hire.
