Job Summary

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team. As an RN Case Manager, you will play a vital role in delivering high-quality patient care and ensuring that our patients receive the best possible outcomes.

Responsibilities
  • Provide comprehensive case management services to patients and their families, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address any identified gaps in care.
  • Monitor patients' progress and adjust care plans as needed to ensure optimal outcomes.
  • Provide education and support to patients and their families to promote health literacy and self-management skills.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practices in case management.
Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Required Certifications and Licensures: Licensed to practice as a registered nurse in the state of agency operation. Must possess and maintain valid CPR certification while employed in a clinical role. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Ability to travel to all business locations.
