We are seeking a skilled RN Care Coordinator to join our team at the University of Texas Medical Branch. As a key member of our interdisciplinary team, you will be responsible for planning, coordinating, and safely transitioning acute care patients to the next level of care.
Responsibilities- Assess patient needs and develop comprehensive discharge plans in collaboration with the healthcare team.
- Utilize leadership principles to mobilize the team and achieve positive outcomes.
- Facilitate communication between physicians, nurses, and ancillary services to ensure seamless care transitions.
- Prepare and implement safe, timely, and effective transition plans for patients.
- Promote a culture of service excellence and quality by implementing and accepting accountability for service standards.
- RN with Associate degree in nursing and two years of clinical nursing experience in an acute hospital environment.
- Current license or valid permit to practice professional nursing in Texas.
- Certification in Care Management (CCM or ACM-RN) to be obtained within three years from date of hire.
Monday - Friday, 8:00 AM - 5:00 PM
