Job SummaryWe are seeking an outstanding Registered Nurse (RN) to join our team in a full-time capacity. As a Registered Nurse Case Manager, you will be responsible for conducting assessments of incoming patients, managing your own case load, and providing symptom management and overseeing patient care while collaborating with our Hospice team members.
Key Responsibilities- Conduct assessments of incoming patients to identify their needs and develop individualized care plans.
- Manage your own case load and conduct visits to patients in their place of residence.
- Provide symptom management and oversee patient care while collaborating with Hospice team members.
- Participate in an on-call rotation for evenings and weekends to ensure 24/7 coverage.
- Have knowledge and understanding of all services hospice provides and keep patients and families informed of all benefits available.
- Educate patients and families on disease progression and provide care coordination and management of patient and family needs related to Hospice diagnosis.
- Deliver outstanding bedside care and foster relationships with patients and families to make a difference in their greatest time of need.
- Valid Registered Nurse (RN) license.
- 1-2 years of nursing experience (preferred).
- Driver's license, insurance, and vehicle.

- Bilingual (English & Spanish).
