Position Overview

We are seeking a dedicated and empathetic Social Worker to join our team. This full-time position focuses on providing support and advocacy for our residents in both skilled and assisted living settings. The ideal candidate will be passionate about enhancing the quality of life for older adults and possess the skills necessary to navigate the complexities of their needs.

Key Responsibilities
  • Conduct comprehensive assessments to identify residents' needs and develop personalized care plans.
  • Provide counseling and emotional support to residents and their families, promoting emotional well-being and resilience.
  • Coordinate with healthcare providers, families, and other team members to ensure comprehensive care and support.
  • Assist residents with accessing community resources and benefits, empowering them to maintain their independence.
  • Maintain accurate and confidential records of resident interactions and progress, ensuring compliance with regulatory requirements.
  • Advocate for residents' rights and well-being within the community, promoting a culture of respect and dignity.
Qualifications
  • Bachelor's or Master's degree in Social Work (BSW or MSW) required.
  • Valid Social Work license (if applicable).
  • Experience in a skilled or assisted living environment preferred.
  • Strong interpersonal and communication skills, with the ability to build rapport with residents, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a patient-centered approach to care.
Schedule and Benefits
  • Full-Time, Monday through Friday.
