Job Summary:

This role involves providing patient-centered care as a member of our interdisciplinary team. You will collaborate with healthcare professionals to develop and implement individualized care plans, ensuring seamless transitions throughout the continuum of care.

Key Responsibilities:

  • Collaborate with patients, families, and healthcare teams to identify and prioritize care needs.
  • Develop and implement effective care plans, taking into account patient preferences, values, and cultural backgrounds.
  • Foster strong relationships with community resources, healthcare providers, and social services to ensure timely access to care.
  • Maintain accurate and detailed records of patient interactions, care plans, and outcomes.

Requirements:

  • Nursing degree/diploma
  • Alaska Registered Nurse License upon hire
  • 2 years Clinical nursing experience
