Job Summary


At Providence Health & Service, we are seeking a skilled Counselor for Multidisciplinary Care Coordination to join our team. This is an exciting opportunity to provide exceptional care to patients and their families/caregivers.


About the Role


The Counselor for Multidisciplinary Care Coordination will work closely with our multidisciplinary team to ensure comprehensive care planning and coordination. This includes collaborating with healthcare providers, social workers, and other specialists to address the social and emotional needs of our patients.


Responsibilities


Key responsibilities of this position include:



  • Developing and implementing individualized care plans to meet the unique needs of patients and their families/caregivers
  • Collaborating with healthcare providers to ensure seamless care coordination and communication
  • Providing emotional support and education to patients and their loved ones
  • Identifying and addressing social determinants of health to improve patient outcomes
  • Participating in multidisciplinary team meetings to discuss patient care and develop strategies for improvement

Requirements


To be successful in this role, you will need:



  • A Master's degree in Social Work (MSW) or a related field
  • Experience working in healthcare, preferably in a multidisciplinary team setting
  • Strong communication and interpersonal skills
  • Ability to work collaboratively with diverse stakeholders, including patients, families/caregivers, and healthcare providers
  • Knowledge of social determinants of health and their impact on patient outcomes
