POSITION SUMMARY:

The Healthcare Respite Support Specialist is responsible for delivering direct assistance, overseeing daily routines, and facilitating activities for individuals enrolled in the Medical Respite program at Siena Francis House. This role is pivotal in aiding clients to achieve their health and wellness objectives outlined in their care plans.

KEY RESPONSIBILITIES:

  • Conduct intake assessments, including a review of medical respite and shelter guidelines.
  • Provide ongoing supervision and support to clients participating in the medical respite initiative.
  • Assist in transporting clients to medical appointments and other necessary engagements.
  • Ensure the medical respite unit is maintained in a clean and sanitary condition.
  • Complete all required documentation accurately and in a timely manner.
  • Observe client behaviors and report any significant changes in their conditions.
  • Collaborate effectively with healthcare professionals, case managers, and shelter staff to support clients in meeting their care plan objectives.
  • Assist clients with their discharge and transition processes.
  • Maintain thorough documentation and data entry as mandated, including progress notes and system entries.
  • Deliver exceptional customer service, fostering an environment of respect and teamwork.
  • Implement organizational policies and procedures to effectively serve clients in the Medical Respite program.
  • Participate in relevant meetings, training sessions, and conferences as required.
  • Communicate clearly and respectfully with clients, their families, colleagues, and community partners, ensuring cultural sensitivity in all interactions.
  • Adhere to confidentiality standards and requirements.
  • Maintain consistent attendance in accordance with applicable standards.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Understanding of issues related to medical respite and homelessness, including mental health, substance abuse, and social challenges.
  • Ability to respond swiftly and calmly in emergencies and recognize symptoms of common health issues.
  • Familiarity with local community resources and support services.
  • Experience working with diverse populations and various levels of staff.
  • Strong communication skills, both verbal and written.
  • Capacity to work independently in a dynamic environment while maintaining organization and attention to detail.
  • Commitment to ongoing development of resilience skills.
  • Keep an updated database of available services and programs.

EDUCATION AND EXPERIENCE:

  • Experience working with individuals facing homelessness or dealing with substance abuse or mental health issues is preferred.
  • High School diploma or equivalent is required.
  • Basic computer proficiency is necessary.

WORK ENVIRONMENT:
