POSITION SUMMARY:

The Healthcare Respite Support Specialist is responsible for delivering direct assistance, overseeing daily activities, and ensuring the well-being of clients enrolled in the Medical Respite program at Siena Francis House. This role is pivotal in helping individuals achieve their health and wellness objectives outlined in their care plans.

KEY RESPONSIBILITIES:

  • Conduct intake sessions, which include reviewing expectations for both medical respite and shelter services.
  • Provide direct oversight and assistance to clients participating in the medical respite program.
  • Facilitate transportation for clients to various appointments.
  • Maintain a clean and sanitary environment within the medical respite unit.
  • Ensure timely and accurate documentation of client interactions and care.
  • Observe client behaviors and report any significant changes in their condition.
  • Collaborate effectively with healthcare professionals, case managers, and shelter staff to support clients in meeting their care plan objectives.
  • Assist clients with their discharge and transition processes.
  • Complete all necessary documentation and data entry promptly, including progress notes and management system entries.
  • Deliver exceptional customer service, fostering a culture of respect and teamwork.
  • Implement organizational policies and procedures to serve Medical Respite clients effectively.
  • Participate in relevant meetings, training sessions, and conferences as required.
  • Communicate clearly and respectfully with clients, their families, and other stakeholders, ensuring cultural sensitivity in all interactions.
  • Adhere to confidentiality standards and client privacy regulations.
  • Maintain regular attendance in accordance with organizational standards.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Understanding of issues related to medical respite and homelessness, including mental health, substance abuse, and domestic challenges.
  • Ability to respond swiftly and calmly in emergencies, recognizing symptoms of common health issues and making appropriate referrals.
  • Familiarity with local community resources and services available to clients.
  • Experience working with diverse populations and varying levels of individuals within a professional setting.
  • Strong communication skills, both verbal and written.
  • Capacity to work independently in a dynamic environment while maintaining attention to detail.
  • Commitment to ongoing personal and professional development.
  • Keep an updated database of available services and programs.

EDUCATION AND EXPERIENCE:

  • Preferred experience working with individuals facing homelessness or struggling with substance use or mental health issues.
  • High School diploma or equivalent is required.
  • Basic computer proficiency is necessary.

WORK ENVIRONMENT:
The characteristics of the work environment are representative of those encountered while performing the essential functions of this role. The setting is residential, with varying temperatures and noise levels.
