POSITION SUMMARY:

The Healthcare Respite Support Specialist is responsible for delivering direct assistance, overseeing daily routines, and facilitating activities for individuals enrolled in the Medical Respite program at Siena Francis House. This role is crucial in aiding clients to achieve their health and wellness objectives outlined in their care plans.

KEY RESPONSIBILITIES:

  • Conduct intake assessments, including a review of expectations for medical respite and shelter services.
  • Provide direct oversight and support to clients participating in the medical respite program.
  • Facilitate transportation for clients to medical appointments.
  • Ensure the medical respite unit is maintained in a clean and sanitary condition.
  • Complete documentation accurately and in a timely manner.
  • Observe client behaviors and report any significant changes in their conditions.
  • Collaborate effectively with healthcare professionals, case managers, and shelter staff to assist clients in meeting their care plan goals.
  • Support clients in their discharge and transition processes.
  • Maintain thorough documentation and data entry as required, including progress notes.
  • Deliver exceptional customer service, fostering a culture of respect and teamwork.
  • Implement organizational policies and procedures to effectively serve clients in the Medical Respite program.
  • Participate in relevant meetings, training sessions, and conferences as needed.
  • Communicate clearly and respectfully with clients, their families, and colleagues, ensuring cultural sensitivity in all interactions.
  • Adhere to all confidentiality standards and requirements.
  • Maintain consistent attendance in accordance with applicable regulations.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Understanding of issues related to medical respite and homelessness, including mental health challenges, substance abuse, and domestic violence.
  • Ability to respond swiftly and calmly in emergencies, recognizing symptoms of common mental and physical health issues and making appropriate referrals.
  • Familiarity with local community resources and support services.
  • Capability to engage with diverse populations and various levels of personnel.
  • Strong communication skills, both verbal and written.
  • Ability to work independently in a dynamic environment while maintaining attention to detail.
  • Commitment to developing resilience skills and practices.
  • Keep an updated directory of available services and programs.

EDUCATION AND EXPERIENCE:

  • Prior experience working with individuals experiencing homelessness or those with substance abuse or mental health issues is preferred but not mandatory.
  • High School diploma or equivalent is required.
  • Basic computer proficiency is necessary.

WORK ENVIRONMENT:
The characteristics of the work environment are typical of those encountered in a residential setting, with varying temperatures and noise levels.
