POSITION SUMMARY:

The Healthcare Respite Program Coordinator is responsible for overseeing the coordination of all activities related to the medical respite program. This role involves collaboration with the Medical Respite Team (MRT) and engagement with the National Institute on Medical Respite Care (NIMRC), ensuring thorough screenings and evaluations of all medical respite referrals.

KEY RESPONSIBILITIES:

  • Supervise and provide coordination support for the medical respite team, including case managers and support technicians.
  • Act as a liaison between local healthcare facilities and the organization.
  • Collaborate with case managers to perform individual needs assessments and develop care plans.
  • Evaluate and review all referrals submitted by healthcare staff.
  • Assess eligibility and appropriateness for enrollment in the medical respite program.
  • Maintain and update the medical respite outcomes tracker to ensure accurate data management.
  • Engage in Health and Housing Coalition meetings, offering updates on medical respite initiatives.
  • Participate in technical assistance activities as needed.
  • Work collaboratively with healthcare professionals, case managers, and shelter staff to assist clients in achieving their goals.
  • Support case managers in creating discharge and transition plans for clients.
  • Deliver exceptional customer service, fostering an environment of collaboration, respect, and teamwork.
  • Implement organizational policies and procedures to effectively serve clients in the medical respite program.
  • Attend relevant meetings, training sessions, and conferences as required.
  • Communicate effectively in both oral and written forms with clients, families, and colleagues, maintaining cultural sensitivity and respect in all interactions.
  • Adhere to all confidentiality standards and requirements.
  • Maintain consistent attendance in accordance with applicable standards.
  • Perform additional duties as assigned.

QUALIFICATIONS:

  • Exceptional organizational skills.
  • Understanding of medical respite care, patient-centered care, and issues related to homelessness, including mental health, substance abuse, and domestic violence.
  • Strong interpersonal skills with the ability to collaborate across diverse organizations and backgrounds.
  • Willingness to quickly learn new software and adapt to changing information needs.
  • Proficient in Microsoft Office Suite, including Word, Excel, and Outlook.
  • Familiarity with local community resources and support services.
  • Ability to work effectively with diverse populations.
  • Excellent verbal and written communication skills.
  • Capacity to work independently in a dynamic environment with a keen attention to detail.
  • Commitment to ongoing personal and professional development.
  • Maintain an updated resource database related to available services.

EDUCATION & EXPERIENCE:

  • A Master's Degree in Social Work is required.
  • Effective communication skills for relaying case information to clients, families, and healthcare professionals.
  • Strong background in social services.
  • Preferred two years of experience in healthcare and case coordination.
  • Basic proficiency in Windows applications and internet usage.

WORK ENVIRONMENT:
This position is primarily office-based, with varying temperature and noise levels.
