Job Summary:
Puget Sound Home Health is seeking an experienced Registered Nurse to join our team as a Hospice Nurse, providing high-quality patient care and support to individuals and their families in Thurston, Grays Harbor, and Mason Counties.
About the Role:
This rewarding position involves working closely with patients and their loved ones to develop personalized care plans that address their unique needs and preferences. As a Hospice Nurse, you will be responsible for conducting comprehensive assessments, implementing treatment plans, and providing emotional support and guidance to patients and their families.
Key Responsibilities:
- Conduct initial and ongoing assessments of patients and their families to determine hospice needs
- Develop and implement personalized care plans that address physical, emotional, and spiritual needs
- Provide skilled nursing care, including medication management and wound care
- Collaborate with interdisciplinary teams to coordinate patient care and services
- Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
Requirements:
- Current licensure as a Registered Nurse in the state of Washington
- Minimum two years of experience in hospice or palliative care nursing, preferably in a home health setting
- Excellent communication, assessment, and problem-solving skills
- Able to work independently with minimal supervision and prioritize multiple tasks efficiently
