Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Hospice Visiting Nurse in Tacoma, Washington. This rewarding role will involve providing comprehensive nursing care to patients in their homes, working collaboratively with an interdisciplinary team to deliver high-quality patient-centered care.

Key Responsibilities
  • Communication and Collaboration: Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
  • Care Planning and Coordination: Assess patient needs, develop individualized care plans, and implement evidence-based interventions to promote optimal health outcomes.
  • Patient Education and Support: Provide education and support to patients and families on disease management, self-care activities, and home safety practices.
  • Skilled Nursing Interventions: Perform complex procedures, such as injections, dressings, and IV care, as part of the home healthcare continuum.
  • Case Management: Work collaboratively with the interdisciplinary team to coordinate patient care, manage resources, and achieve quality outcomes.
Requirements
  • Education: Associate degree in Nursing from an accredited program.
  • Licensure: Current Registered Nurse License (Washington) required at hire.
  • Experience: Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings.
  • Skill Set: Basic laptop skills, MS Office Suite proficiency, customer service skills, and ability to work independently.

Compensation and Benefits

The salary range for this position is $95,000 - $125,000 per year, depending on experience and qualifications.
