Hospice RN Case Manager Specialist

We are seeking a compassionate and skilled Hospice Registered Nurse (RN) to join our team at Envision Hospice. As a Hospice RN, you will play a critical role in providing high-quality care to patients with terminal illnesses.
About the Role:
This position involves working in various residential settings, including private homes, assisted living facilities, and skilled nursing facilities. You will be responsible for assessing patient needs, developing personalized care plans, and implementing interventions to improve patient outcomes.
Key Responsibilities:
- Conduct thorough assessments of patients' physical, emotional, and social needs.
- Develop and implement individualized care plans that address patients' unique needs.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
- Provide education and support to patients and their families.
- Manage symptoms, medications, and other aspects of care to optimize patient outcomes.
Requirements:
- Master's degree in nursing or related field.
- Current RN licensure in Washington state.
- Minimum 2 years of experience as a Hospice RN or similar role.
- Excellent communication and interpersonal skills.
- Ability to work independently and make sound judgments.
