Job Summary

We are seeking a compassionate and skilled LPN to join our team as a Home Health Hospice Caregiver in Tacoma. As a key member of our interdisciplinary care team, you will provide direct in-home nursing care to clients, working under the direction and supervision of a registered nurse or licensed physician.

Essential Responsibilities:

  • Assess patient/consumer need for care and communicate with RN/MD providers accordingly.
  • Determine patient/consumer purpose for admission/visit and collect relevant data.
  • Recognize urgent/emergent situations and refer/manage appropriately.
  • Contribute to the plan of care and assist in monitoring care.
  • Deliver nursing care as determined by the plan or patient needs.
  • Recommends modifications in the plan of care as needed.

Nursing Interventions:

  • Provide safe and competent nursing interventions and report relevant findings to RN/MD in a timely manner.
  • Follow through and implement the plan of care as directed.
  • Administer approved medications at the direction of the RN and upon order from MD, ARNP, or PA.
  • Perform treatments and assist with procedures as directed.

Evaluation and Communication:

  • Assist in evaluating patient/consumer response to care through communication and observation.
  • Recommends modifications in the plan of care to RN/MD/ARNP/PA based on patient/consumer response.
  • Communicate and cooperate with other healthcare team members in planning and implementing care.

Age-Specific Care:

  • Demonstrate understanding of the physiologic and psychological effects of normal aging by assessing and implementing care to meet the needs of those populations served.
  • Report appropriate age-specific assessment findings to the RN.

Salary Information:

We offer a competitive salary range of $ $73,900 - $96,300 per year, depending on experience, which is commensurate with industry standards for this role and location.

Basic Qualifications:

  • Experience: Minimum one (1) year of home health experience. Per the National Agreement, current KP Coalition employees have this experience requirement waived.
