We are seeking a compassionate Licensed Practical Nurse to join our team at Envision Hospice in Pierce and Thurston County, WA.

Job Summary:

As a Patient Care Coordinator, you will provide exceptional care to patients who require one-to-one attention and monitoring in their homes, assisted living facilities, and skilled nursing facilities.

Responsibilities:

  • Conduct thorough client assessments and develop personalized patient care plans
  • Follow physician orders and maintain compliance with state regulations and company policies
  • Deliver high-quality patient care while demonstrating empathy and integrity
  • Communicate effectively with patients, families, and healthcare teams
  • Provide one-to-one care in various settings, prioritizing patient needs and comfort

Requirements:

  • Graduate of an accredited nursing school
  • Licensed LPN in the State of Washington
  • At least 1 year of experience in acute care or home care/hospice setting (preferred)
