Transitional Support Coordinator

The Catholic Charities Maine Behavioral Health Network seeks a dedicated Transitional Support Coordinator to join its interdisciplinary team. As a key member of the Client Access Response and Engagement (C.A.R.E.S.) Team program, you will provide vital transitional support services to individuals and families navigating mental health diagnoses and substance use disorders. This dynamic role requires a highly organized and resourceful individual with experience working in behavioral health settings. As a Transitional Support Coordinator, you will bridge supports to individuals enrolled in various BHN programs, while maintaining a caseload of clients requiring assessment, stabilization support, or life transitions. Your direct support will include requests for services, pre-intake and orientation, access to services, conducting outreach, and provisioning services to individuals on your assigned caseload.

In this impactful role, you will collaborate with a diverse team of professionals to provide an integrated approach to client care. As a Transitional Support Coordinator, you will have the opportunity to make a lasting difference in the lives of those we serve.

About the Position

This full-time position offers a competitive salary of $58,000 - $68,000 per year, depending on experience. Benefits include:

Qualifications

To excel in this role, you will need to meet the following qualifications:

* Certified by the Department as a Mental Health Rehabilitation Technician/Community (MHRT/C or provisional A, B with work actively being completed toward C)
* Motivation to obtain a CADC and CRMA (or ability to obtain within 6-months of hire)
* Experience working in behavioral health settings with adults, youth, and families
* At least two years of relevant experience working with adults and/or youth and families, specifically in a behavioral health case management capacity
* Knowledge of pertinent local community resources
* Proficiency in computer skills necessary to function successfully in an electronic health record environment
