About the Role

The Transitional Care Coordinator will work in a collaborative capacity, bridging supports to individuals enrolled in all Behavioral Health Network programs. This role involves providing direct support to requests for services, pre-intake and orientation, access to services, conducting outreach, and provisioning services to individuals on their assigned caseload.

Responsibilities
  • Provide care coordination services within the Client Access Response and Engagement (C.A.R.E.S.) Team program
  • Work collaboratively with the C.A.R.E.S. team to bridge supports to individuals enrolled in all BHN programs
  • Support a caseload of individuals requiring assessment, stabilization, or service transitions
Requirements

To be successful in this role, you must have:

  • A working knowledge of pertinent local community resources
  • The requisite computer skills necessary to function successfully in an electronic health record environment
  • A minimum of two years of relevant experience working with adults and/or youth and families in a behavioral health case management capacity
  • Current certification as a Mental Health Rehabilitation Technician/Community (MHRT/C) or provisional A, B with work actively being completed toward C
