About the Role

The Healthcare Delivery Coordinator role is an exciting opportunity to join our team at Maine Medical Center. As a key member of our healthcare delivery team, you will be responsible for coordinating patient care activities, communicating effectively with patients, families, and healthcare team members, and ensuring seamless transitions of care.

**Salary:** $60,000 - $85,000 per year (based on experience)

In this role, you will have the opportunity to work with a talented team of healthcare professionals who are dedicated to delivering exceptional patient care. You will be responsible for prioritizing tasks, managing multiple projects simultaneously, and maintaining accurate records and documentation.

Responsibilities:

* Coordinate patient care activities, including scheduling appointments, preparing patients for procedures, and escorting patients to exams
* Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions of care
* Maintain accurate records and documentation, including electronic medical records and other relevant data systems
* Prioritize tasks, manage multiple projects simultaneously, and meet deadlines

Requirements:

* Graduate of an accredited School of Nursing required; BSN preferred
* Current applicable state(s) license as a Registered Professional Nurse required
* Current BLS certification required or must obtain within 30 days of start date
* Two years of RN experience required; three years of relevant experience preferred
