As a Home Health Care Coordinator, you will be responsible for delivering high-quality patient care in the comfort of their own homes. This involves assessing patient needs, developing personalized care plans, and collaborating with other healthcare professionals to ensure seamless care transitions.

**Key Qualifications:

- Graduation from an accredited school of nursing with a current license to practice in Maine.

- At least one year of experience as a Registered Nurse in a hospital or home care setting.

- Strong understanding of nursing principles, including assessment, diagnosis, planning, implementation, and evaluation.

- Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
