Position Summary:


As a Home Health Clinical Manager/RN, you will play a crucial role in coordinating and overseeing patient care to ensure the highest standards are upheld.

This role includes supervising clinical staff and ensuring the provision of quality home care services.


Key Responsibilities:
  • Receive case referrals and assess patient needs to assign suitable clinicians.
  • Review and evaluate each case, providing guidance to clinicians for optimal performance.
  • Instruct and support clinicians to promote quality care delivery, being available to assist as necessary.
  • Review patient clinical information, including diagnoses, medications, and procedures.
  • Assist in establishing therapeutic goals and developing care plans.
  • Participate in case conference meetings to facilitate care coordination.
  • Conduct quarterly record reviews and communicate findings to relevant personnel.
  • Assist in screening, interviewing, and orienting new staff.
  • Help plan and implement in-service and continuing education programs.
  • Contribute to the formulation, revision, and implementation of policies and procedures.
  • Perform direct patient care duties as required.
  • Maintain compliance with professional standards and principles.

Required:

  • A minimum of 2 years of experience in home health care
  • At least 1 year of management or supervisory experience, preferably in home health
