Position Overview: The Case Manager will work closely with low-income and homeless veterans and their families, providing essential support and guidance.

Salary: $52,000 per year

Role Summary: We are looking for passionate Case Managers who are committed to assisting veterans and their families during challenging times. As a Case Manager, you will play a vital role in enhancing the well-being and stability of veterans facing homelessness, housing instability, or residing in subsidized housing. Our programs aim to support individuals in achieving their goals and wellness through a Housing First, client-centered, and trauma-informed approach. In collaboration with our Veteran's service team, you will positively impact the lives of clients by fostering supportive relationships and connecting them to necessary resources for securing or maintaining permanent housing.

Key Responsibilities:

  • Manage a caseload of veteran clients.
  • Collaborate with households to create housing plans and budgets, assisting veterans in accessing resources for permanent housing.
  • Support veteran households in increasing income and preparing them for independent housing maintenance and financial stability.
  • Conduct assessments and coordinate services with VA Medical Centers and community agencies.
  • Guide households in applying for housing and subsidy programs, as well as state and federal benefits.
  • Maintain consistent communication with veteran households and community partners.
  • Identify and address barriers to permanent housing and conduct inspections for potential housing units.
  • Educate clients on tenant rights and responsibilities.
  • Engage in supervision, team meetings, audits, training, and professional development.
  • Adhere to professional standards, ensuring all services are documented and client records are maintained.

Qualifications:

  • BA/AA degree preferred; alternatively, a minimum of 2 years of relevant experience in case management may be accepted.
  • Strong understanding of veteran issues, homelessness, and available state and federal resources is highly desirable.
  • Proficiency in electronic health records, Microsoft Word, and Outlook is preferred.
