Job Summary

Key Responsibilities
  • Provide clinical support to the management team and coordinate the delivery of care to patients and families in their homes.
  • Triage calls related to patient care, including physician orders, staff issues, and other clinical concerns.
  • Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including direct patient care.
  • Communicate regularly with Clinical Managers and the Clinical Director on clinical practice and performance-related issues.
  • Demonstrate expertise and knowledge in areas such as current methods of care and treatment, policies, procedures, and protocols specific to the agency, infection control, reimbursement criteria, and regulations.
  • Assist with individual caseload reviews, audits, and orientation of clinical staff.
  • Evaluate performance of staff in relation to established standards, determine staff competency, and provide direction as needed.
  • Work collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
  • Triage calls from physicians and follow through on required actions, including communication to field staff, changes in orders, and scheduling.
  • Provide documentation that is concise, accurate, and informative, and supports reimbursement.
  • Practice in accordance with standards of care, policies, procedures, and scope of practice, demonstrating awareness of regulatory guidelines.
Requirements
  • Registered Nurse (RN) license
  • Three years of community health experience, or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience
