Key Responsibilities- Provide clinical support to the management team and coordinate the delivery of care to patients and families in their homes.
- Triage calls related to patient care, including physician orders, staff issues, and other clinical concerns.
- Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including direct patient care.
- Communicate regularly with Clinical Managers and the Clinical Director on clinical practice and performance-related issues.
- Demonstrate expertise and knowledge in areas such as current methods of care and treatment, policies, procedures, and protocols specific to the agency, infection control, reimbursement criteria, and regulations.
- Assist with individual caseload reviews, audits, and orientation of clinical staff.
- Evaluate performance of staff in relation to established standards, determine staff competency, and provide direction as needed.
- Work collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
- Triage calls from physicians and follow through on required actions, including communication to field staff, changes in orders, and scheduling.
- Provide documentation that is concise, accurate, and informative, and supports reimbursement.
- Practice in accordance with standards of care, policies, procedures, and scope of practice, demonstrating awareness of regulatory guidelines.
- Registered Nurse (RN) license
- Three years of community health experience, or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience
