POSITION SUMMARY OF THE LIFE SKILLS ASSOCIATE PROGRAM COORDINATOR

The Life Skills Associate Program Coordinator plays a crucial role in assisting individuals with intellectual and developmental disabilities (I/DD) in reaching their utmost level of independence. This position emphasizes a person-centered care approach, ensuring that each Associate Program Coordinator delivers support and guidance aligned with the defined service objectives to achieve optimal outcomes.

A key responsibility of the Associate Program Coordinator is to collaborate with the Program Coordinator to cultivate a unified team environment. The Associate Program Coordinator is expected to motivate others, inspire enthusiasm, and lead by example, making a significant positive impact through effective relationship building.

KEY RESPONSIBILITIES

  1. Operational Duties
  • Oversee the health and safety of all participants.
  • Assist in monitoring and executing individual support plans.
  • Administer medications (certification required).
  • Participate in support plan and individual program meetings as necessary.
  • Provide administrative support to the Program Coordinator and instructors.
  • Supervise custodial staff responsibilities.
  • Step in as an instructor when required.
  • Report maintenance issues and assist in resolving them.
  • Ensure daily classes are inclusive, engaging, and interactive.
  • Understand and implement behavior plans.
  • Identify individuals with behavior plans and communicate protocols to relevant instructors.
  • Establish and maintain supportive, professional relationships with individuals, their families, and support networks.
  • Deliver individualized services in accordance with service descriptions outlined in the service directory.
  • Create a warm, inviting atmosphere that promotes personal interest and positivity while maintaining a calm environment.
  • Recognize and document any physical or emotional changes in individuals, seeking appropriate interventions and reporting changes promptly.
  • Assist individuals with meals, personal care, transitions, and other tasks as needed.
  • Support daily arrival and departure tasks.
  • Manage supply inventory and communicate needs to the Program Coordinator to ensure the smooth operation of individual support plans and classes.
  • Document and submit all incident reports following guidelines.
  • Communicate all incidents to the Program Coordinator.
  • Assist with content creation for social media.
  • Organize community outings.
  • Ensure the facility is cleaned and sanitized according to safety standards.
  1. Administrative Responsibilities
  • Complete all required documentation, including daily notes and summaries on progress towards support plan goals.
  • Immediately report any difficulties, accidents, incidents, or concerns to the Program Coordinator.
  • Prepare General Event Reports using the designated documentation system and submit to necessary parties.
  • Enter detailed notes daily into the selected documentation system.
  1. Additional Duties
  • Assist individuals with boarding and alighting from transportation.
  • Serve as a positive role model for participants through appropriate conduct and demeanor.
  • Plan and implement activities that align with individual program goals, enrichment, and physical activity using provided curriculum and materials.
  • Adhere to company policies and procedures.
  • Maintain confidentiality and comply with HIPAA regulations.
  • Attend all required training sessions as directed by the employer.
  • Perform other projects and duties as assigned.

SUPERVISORY ROLE

  • No supervisory responsibilities.

WORKING ENVIRONMENT AND PHYSICAL DEMANDS

  • Primary activities are conducted in a well-lit, climate-controlled environment.
  • Occasional community outings may involve outdoor travel and exposure to varying weather conditions.
  • Ability to bend, stand, walk, or sit for extended periods and possess a normal range of motion.
  • Must be able to lift up to 50 pounds independently.
  • Ability to provide personal hygiene assistance to individuals as needed.
  • Potential exposure to bodily fluids and other pathogens.
