Job Title: Quality Care Coordinator

Arkos Health is seeking a highly skilled Quality Care Coordinator to join our team. As a Quality Care Coordinator, you will play a critical role in ensuring the highest quality of care for our patients.

Job Summary:

The Quality Care Coordinator will be responsible for conducting outreach to our member population to encourage healthy behaviors through innovative telephonic communications and completing in-home visits to help meet patients' needs. You will also enter data into our database for all CPT II codes being performed and schedule and follow up on clinical care needs for patients as it relates to HEDIS and other quality STAR initiatives.

Key Responsibilities:
  • Review data collected to confirm gaps in care are captured
  • Coordinate and order preventative and diagnostic tests around gaps in care on behalf of the primary care physician utilizing standing orders/protocols required by the health plan
  • Conduct data entry and maintain appropriate documentation and tracking for quality initiatives
  • Conduct outreach to specified patients on priority lists for any outstanding care gaps and schedule accordingly
  • Educate patients on procedures that are being scheduled/performed and the reason for performing them
  • Perform in-home visits with patients
  • Review data collected to confirm gaps in care are captured
  • Build positive, professional relationships with the patients we serve
  • Explain point-of-care testing and reasoning to patients
Requirements:
  • High school diploma or GED equivalent required
  • Must have valid Arizona driver license and appropriate liability insurance
  • Medical Assistant (MA) Certification and or Certified Nurse Assistant (CNA) Certification (Required)
  • Current CPR required
  • 2+ years' experience in a healthcare environment
  • Experience with Microsoft Word, Excel, and Power Point
  • Experience in Electronic Health Records
  • Ability to collect and process specimen samples
  • Experience with verifying Eligibility and navigating through Insurance Portals
  • Experience with CPT II codes
  • Basic knowledge HCC's and Medical Diagnosis
  • Ability to perform multiply procedures (i.e.) ABI, DRE, Urine Dips if necessary
  • Familiarity of Medicare guidelines to perform patient screening's
