Treatment Coordinator Job Description

Jewish Family & Children's Service (JFCS) is seeking a skilled Treatment Coordinator to join our team. As a Treatment Coordinator, you will play a vital role in providing ongoing care-coordination services to assist in addressing the client and family's ongoing service needs.

Key Responsibilities:
  • Service Coordination: Perform ongoing coordination of services to children and adults in accordance with the treatment plan.
  • Crisis Response: Respond to initial crisis calls and identify needs & necessary resources accordingly.
  • Resource Engagement: Use established best practices to engage formal and informal resources for the client, including the transition, discharge, and aftercare plans of behavioral-health services.
  • Collaboration: Provide coordination of behavioral-health services with other agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, schools, and other providers.
Qualifications:
  • Education: Bachelor's Degree in behavioral health field preferred; a combination of experience and education may be considered.
  • Experience: Relevant experience working in a behavioral-health setting.
  • Certifications: Level 1 Fingerprint Clearance Card.
  • Verification: Verification of education, employment, references.
  • Driving Requirements: MVR matching JFCS driving guidelines.
  • License: AZ Driver's License and Valid Auto Insurance.
