Job Overview

Position Title: Housing Care Manager

POSITION SUMMARY

The Housing Care Manager is an integral part of the St. John's Community Health Housing Program team, dedicated to assisting Medi-Cal members enrolled in the program with essential housing and support services. This role encompasses a variety of Housing Navigation services, including client advocacy, comprehensive case management, and connecting individuals to mental health and substance abuse resources, as well as stable housing solutions and other necessary support services.

Key Responsibilities:

  • Deliver Housing Navigation services to Medi-Cal members, ensuring successful tenancy by:
    • Conducting thorough Housing Assessments to identify client preferences and barriers.
    • Creating tailored Individualized Housing Support Plans that address obstacles and set achievable housing goals.
    • Facilitating the housing application process and securing vital documentation.
    • Advocating for benefits and assisting with the SSI application process.
    • Identifying and securing financial resources to assist with rent and other housing-related expenses.
    • Communicating effectively with landlords on behalf of clients.
    • Establishing procedures to maintain housing stability.
    • Coordinating transportation solutions to ensure access to housing options.
    • Maintaining accurate program records and documentation.
  • Provide Tenancy Services to ensure ongoing support for clients after housing is secured, including:
    • Identifying and addressing behaviors that may threaten housing stability.
    • Educating clients on tenant rights and responsibilities.
    • Coaching clients on building positive relationships with landlords.
    • Facilitating conflict resolution between clients and landlords or neighbors.
    • Assisting with annual housing recertification processes.
    • Regularly reviewing and updating Individualized Housing Support Plans to reflect current needs.
    • Offering guidance on independent living skills, including budgeting and financial literacy.

Qualifications:

  • A Bachelor’s degree or higher is required.
  • Familiarity with resources that promote stable housing, including Veteran Services and Rapid Rehousing.
  • Proficient in housing navigation skills, including application processes and lease compliance.
  • Minimum of 2 years of experience in social services or public health.
  • Experience working with diverse and underserved communities.
  • A valid driver's license and vehicle for travel are necessary.
  • Bilingual proficiency in English and Spanish is strongly preferred.
