Windsor Health & Rehabilitation Center LLC is looking for a Clinical Assessment Coordinator to enhance our team. In this role, you will collaborate with a variety of healthcare professionals to ensure outstanding patient care. The Clinical Assessment Coordinator is tasked with collecting vital information regarding the current and prospective patients of a healthcare facility, focusing on both physical and psychological well-being. This position involves reviewing patient records and liaising with healthcare teams to develop suitable clinical care strategies for both existing and incoming residents.

Key Responsibilities:

  • Evaluate potential Patient Driven Payment Method (PDPM) and associated costs for prospective admissions.
  • Engage in the admission process for new residents, assessing their nursing requirements and determining the appropriate reimbursement level.
  • Oversee the MDS process for all residents, ensuring accuracy and compliance.
  • Maintain up-to-date knowledge of Medicare guidelines, serving as a resource for nursing staff and communicating any regulatory changes.
  • Track Case Mix Index (CMI) scores, identifying potential risks or changes that could impact Medicaid reimbursement.
  • Ensure timely submission and accuracy of Medicare assessments and nursing documentation.
  • Support physicians in delivering continuous patient care.
  • Promote the physical, mental, and nutritional well-being of patients.
  • Relay any changes in patient health status to other medical personnel.
  • Offer preventive health care recommendations to patients and the interdisciplinary team.
  • Participate in standardized healthcare programs, seminars, and educational initiatives.
Qualifications:
  • Prior experience in a similar role as a Clinical Assessment Coordinator.
  • RAC Certification is advantageous.
  • Familiarity with Resource Utilization Groups (RUGs), PDPM, and Case Mix Index.
  • Possession of a current and valid state Registered Nurse license.
  • Ability to establish rapport with both patients and staff.
  • Exceptional written and verbal communication skills.
  • Capacity to excel in a fast-paced environment.
