Job Summary

Network Human Services is seeking a skilled RN Care Coordinator to oversee the health and wellbeing of residents and supervise personnel in regards to medical issues and medication administration.

Key Responsibilities
  1. Ensure compliance with agency and DDS policies, as well as governing regulations, in regards to medical care for residents.
  2. Implement MD orders and ensure they are followed, reporting errors as needed.
  3. Order and administer medications, monitor residents' vitals and weights, and coordinate hospital admissions and discharges.
  4. Track medical appointments, attend resident medical appointments, and collaborate with outside medical professionals.
  5. Ensure special diets and consistencies are followed and prepared properly, and perform daily reviews of individuals' kardexes and MD orders.
  6. Oversee the general health and welfare of day program participants, provide medical assessments as needed, and complete documentation and follow-up as required.
  7. Maintain accurate and orderly medical records, and ensure program compliance with medical protocol through timely and accurate reporting of medical data.
Requirements
  1. Graduate of an accredited Registered Nurse Program and maintains a current Registered Nurse license through the State of CT.
  2. Good interpersonal relationships, verbal communication skills, and ability to handle confidential information.
  3. Ability to handle stressful and/or crisis situations, and good time-management skills.
  4. Must hold a valid CT driver's license and be insurable with Network's insurance carrier.
