Job Summary

CommuniCare Health Services Corporate is seeking a highly skilled Patient Assessment Coordinator to join our team. As a Patient Assessment Coordinator, you will play a critical role in identifying patients for potential admission into our facilities and maintaining positive relationships with current referral sources.

Key Responsibilities
  • Perform Clinical Assessments: Conduct thorough clinical assessments to identify patients who may benefit from our services.
  • Resource to Liaisons: Provide support and guidance to our liaison team to ensure seamless communication with referral sources.
Requirements
  • License and Certification: Current, valid and active license to practice as a Registered Nurse or Licensed Practical Nurse in the state of Ohio, or Social Work Degree, or other medical credential.
  • Experience: Three (3) years treatment experience in a Long Term Care setting preferred.
  • Clinical Screening Experience: Previous clinical screening experience preferred.
  • Healthcare Experience: Recent healthcare experience in Long Term Care preferred.
  • Knowledge of Local Medical Community: Knowledge of local medical community preferred.
  • Customer Service Skills: Strong customer service skills and commitment to service excellence.
  • Communication Skills: Ability to collect, organize, evaluate and communicate professionally and assertively, both verbally and in writing, pertinent clinical information with medical and administrative staffs, patients and families.
  • Organizational Skills: Strong organizational, prioritizing and analytical skills.
  • Decision-Making: Ability to make independent decisions when circumstances warrant.
  • Technical Skills: Working knowledge of computer and software applications used in job functions.
