Job Summary

The Case Manager will be responsible for providing individualized support and services to individuals with disabilities, helping them to achieve their goals and improve their quality of life. This is a rewarding opportunity to make a meaningful difference in the lives of others.

Key Responsibilities

  • Develop and implement personalized plans of care for individuals with disabilities, taking into account their unique needs and goals.
  • Provide advocacy and support to individuals and their families, connecting them with community resources and services.
  • Conduct regular assessments and evaluations to ensure that individuals are receiving the necessary support and services.
  • Collaborate with healthcare providers, social workers, and other professionals to ensure that individuals receive comprehensive care.
  • Maintain accurate and up-to-date records of individual progress and services provided.

Requirements

  • Bachelor's or Master's degree in a human service field, such as psychology, sociology, social work, or counseling.
  • Experience working with individuals with disabilities, preferably in a case management or support planning role.
  • Strong communication and interpersonal skills, with the ability to build rapport with individuals and their families.
  • Ability to work independently and as part of a team, with a high level of flexibility and adaptability.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
