We are seeking a highly skilled Clinical Case Manager to join our team at The Affiliated Sante Group. As a Clinical Case Manager, you will play a critical role in supporting individuals with mental health and behavioral health needs.
Key Responsibilities- Conduct comprehensive assessments to identify client needs and develop individualized rehabilitation plans.
- Provide counseling and support to clients to help them achieve their goals and improve their social functioning.
- Collaborate with community resources and other organizations to ensure clients receive the necessary services and support.
- Coordinate care with local agencies, hospitals, and families to evaluate client capabilities and compile necessary documentation.
- Meet with clients on a monthly basis to establish and maintain a good working relationship.
- Responsible for coordination of care with all agencies, doctors, hospitals, families, etc.
- Identify comprehensive clinical training needs, coordinate, and train staff on a quarterly basis.
- Conduct and write all assessments and dispositions on new clients in Montgomery County.
- Set up, conduct, organize, and update treatment team meetings (IRP's or Interim) for RRP clients and Supported Housing clients.
- Submit follow up forms to the Community Forensics Aftercare monthly and quarterly for clients on conditional releases.
- Compile, maintain, audit and purge clinical records of consumers in all locations per agency policy and state regulations.
- Write clinical documentation as needed (authorizations, quality assurance, incident reports, contact notes, etc.).
- Ensure client travel is established and maintained.
- Submit all required maintenance orders for client sites.
- Attend all mandatory meetings and record minutes.
- Provide 24-7 on call service for clinical emergencies.
- Participates in and successfully completes all required trainings.
- Bachelor's Degree with at least three (3) years' related experience; OR at least seven (7) years' related experience.
- Must be able to pass Maryland Board of Nursing CMT.
- Salary: $40,000
- Mileage reimbursement.
