- Coordinate with community resources, healthcare providers, and family members to ensure a smooth transition from hospital to home or other care settings.
- Utilization Management and Quality Improvement
- Monitor patients' progress and identify opportunities for improvement in care delivery.
- Develop and implement strategies to reduce length of stay, readmissions, and other quality metrics.
- Education and Training
- Provide education and training to patients, families, and healthcare professionals on case management principles and best practices.
- Stay up-to-date with industry developments, research, and evidence-based practices to inform case management decisions.
- Education
- Bachelor's degree in nursing or related field.
- Master's degree in nursing or related field preferred.
- Experience
- At least 5 years of experience in case management or a related field.
- 2 years of experience in a hospital or healthcare setting preferred.
- Certifications
- Accredited Case Manager (ACM) certification or equivalent.
- Other relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Management (CPHM), may be preferred.
- Communication and Interpersonal Skills
- Excellent verbal and written communication skills.
- Ability to work effectively with diverse populations, including patients, families, and healthcare professionals.
- Case Management Knowledge and Skills
- Comprehensive knowledge of case management principles, practices, and best practices.
- Ability to assess patients' needs and develop individualized care plans.
- Analytical and Problem-Solving Skills
- Ability to analyze complex data and identify opportunities for improvement in care delivery.
- Strong problem-solving skills to develop and implement strategies to reduce length of stay, readmissions, and other quality metrics.
