Requirements
  • Education
    • Bachelor's degree in nursing or related field.
    • Master's degree in nursing or related field preferred.
  • Experience
    • At least 5 years of experience in case management or a related field.
    • 2 years of experience in a hospital or healthcare setting preferred.
  • Certifications
    • Accredited Case Manager (ACM) certification or equivalent.
    • Other relevant certifications, such as Certified Case Manager (CCM) or Certified Professional in Healthcare Management (CPHM), may be preferred.

  • Communication and Interpersonal Skills
    • Excellent verbal and written communication skills.
    • Ability to work effectively with diverse populations, including patients, families, and healthcare professionals.
  • Case Management Knowledge and Skills
    • Comprehensive knowledge of case management principles, practices, and best practices.
    • Ability to assess patients' needs and develop individualized care plans.
  • Analytical and Problem-Solving Skills
    • Ability to analyze complex data and identify opportunities for improvement in care delivery.
    • Strong problem-solving skills to develop and implement strategies to reduce length of stay, readmissions, and other quality metrics.
