Compensation:

Position Overview:
We are currently seeking a full-time Senior Care Coordinator to support our Elderly Services.Key Responsibilities:
  • Provide supportive services to clients within our Senior Services Department.
  • Conduct geriatric care assessments, case management, care navigation, and resource referrals through one-on-one support sessions, family meetings, and group sessions.
Qualifications:
  • Required: High School Diploma/GED; 3-5 years of experience in social services or case management; experience working with interdisciplinary care teams; experience serving seniors and low-income populations.
  • Preferred: Master’s degree in Social Work with LGSW or LICSW preferred but not required; 3-5 years of experience in senior services; 3 years of case management experience.
Licenses/Certifications:
  • CPR/First Aid certification.
  • LGSW or LICSW preferred (must be licensed in the relevant jurisdiction).
  • Valid Driver’s License.
Schedule:
Monday - Friday, 8:00 am - 4:30 pm (40 hours per week)
Knowledge, Skills, and Abilities:
  • Understanding of healthcare finance standards and procedures for waiver providers.
  • Strong problem-solving skills.

  • Proficient in MS Office applications.

  • Counseling and coaching skills.

Expected Contributions:
  • Act as a liaison between community partners and Senior Services.
  • Collect, interpret, and report data for grant development.
  • Conduct routine geriatric care assessments to identify barriers and treatment goals.
  • Develop person-centered management plans with clients.
  • Provide care coordination expertise through case consultations and psychoeducation.
  • Guide Senior Service teams to ensure best practices and uphold care standards.
  • Assist with oversight of services and daily operations.
  • Coordinate client care with external providers.
  • Provide emergency support services and crisis intervention.
  • Promote compliance with health guidelines.
  • Develop and implement community education curricula.
  • Foster engagement and morale-building initiatives.
  • Receive and coordinate new clinical program referrals.
Intern Responsibilities:
  • Support clinical staff in screening and interviewing MSW intern candidates.
  • Assist in providing direction and guidance to MSW interns.
  • Help define work requirements and assign tasks.
