Key Responsibilities:

  • Perform comprehensive medical evaluations for all clients
  • Develop individualized nursing management strategies tailored to clients' health requirements
  • Facilitate the intake process for clients entering medical management services
  • Maintain accurate documentation of assessments and subsequent care
  • Collaborate with the healthcare team for patient assessments
  • Provide educational resources to patients
  • Manage prescription renewals
  • Monitor patient information and prepare reports
  • Ensure adherence to quality assurance standards
  • Engage in team discussions and case review meetings
  • Comply with established standards for Nurse Case Management services
  • Execute additional assigned responsibilities

Qualifications:

  • Licensed Vocational Nurse with experience in case management
  • Familiarity with working with HIV/AIDS patients is advantageous
  • Capability to operate independently within diverse communities
  • Spanish language proficiency is preferred
  • Competence in computer usage
