Position Objective:

As a Licensed Vocational Nurse (LVN) Case Coordinator within our Enhanced Care Management (ECM) division, you will deliver comprehensive case management and healthcare services to a diverse clientele, including high utilizers of services, individuals and families facing homelessness, adults with serious mental health or substance use challenges, and those reintegrating after incarceration. Your key responsibilities will encompass conducting thorough assessments, formulating care plans, and coordinating medical services for those experiencing homelessness. You will work collaboratively with interdisciplinary teams, including social workers, counselors, and outreach professionals, to meet the multifaceted needs of our clients. Furthermore, you will facilitate access to medical resources, provide health education, and advocate for the well-being of our clients within the community. Your empathy, clinical knowledge, and dedication to serving vulnerable populations will be crucial in assisting individuals experiencing homelessness in accessing quality healthcare and striving for stability and improved health outcomes.

Key Responsibilities:

  1. Collaborate with ECM staff, healthcare providers, managed care organizations, housing navigation teams, and community service providers to ensure enhanced care coordination for eligible patients, facilitating a seamless experience and preventing service duplication.
  2. Conduct case management services, including intake and comprehensive assessments (functional, psychosocial, housing, mental health, and substance use). Develop a detailed care plan based on the assessment with measurable goals, interventions, and objectives for effective implementation. This includes referrals to specialists, ongoing monitoring of the care plan, and necessary follow-ups.
  3. Connect clients to additional social and support services, providing appropriate referrals for specialized care (e.g., ophthalmology, behavioral health, nutrition), transportation, food assistance, and housing. Accompany clients to appointments as needed.
  4. Deliver health education tailored to the client's needs and follow up on compliance issues, particularly regarding medication management, blood sugar, and blood pressure.
  5. Advocate for clients with healthcare professionals.
  6. Utilize motivational interviewing, trauma-informed care, and harm-reduction strategies.
  7. Coordinate with hospital staff on discharge planning.
  8. Monitor treatment adherence, including medication compliance.
  9. Provide health promotion and self-management training.
  10. Collect and input necessary data for reporting purposes. Complete and submit monthly project activity and other reports as required for funding sources.
  11. Participate in agency and community meetings addressing patient needs and issues related to case management.
  12. Attend weekly case conferences with primary care providers and clinical supervisors to assist in problem-solving for clients and ensure the delivery of high-quality case management services.
  13. Perform additional duties as assigned.

Qualifications:

  1. Current LVN license with a minimum of 2 years of experience in case management.
  2. Experience working with individuals with multiple chronic conditions, frequent emergency room visits, mental health issues, substance use disorders, or those experiencing homelessness.
  3. Proficient in Electronic Health Records and data entry; experience with Next Gen preferred.
  4. Skilled in Microsoft Word, Excel, and PowerPoint, with excellent communication and customer service abilities.
  5. Strong commitment to working as part of a multidisciplinary team with care coordinators, RNs, LCSWs, MSWs, and MAs.
  6. Bilingual proficiency (English/Spanish) is highly preferred.
  7. Possess reliable transportation, a valid California Driver's License, and proof of insurance.
  8. BLS/First Aid certification required.
  9. Ability to thrive in a dynamic and fast-paced healthcare environment.
