Chronic Care Licensed Vocational Nurse

Role Summary:
As a Licensed Vocational Nurse specializing in Disease Management, you will operate under the guidance of the Registered Nurse Supervisor at Arroyo Vista Family Health Center, a community-focused non-profit organization. Your primary responsibility will be to manage cases for patients with chronic illnesses, particularly those suffering from diabetes. This role encompasses coordinating patient care, analyzing health data, delivering nursing services, and providing education to patients. You will work collaboratively with a multidisciplinary team to ensure optimal care for diabetic individuals.
Key Responsibilities:
- Oversee the care of patients with chronic conditions, emphasizing diabetes management.
- Gather, assess, and interpret data related to the quality of care for diabetic patients.
- Provide nursing services across various age groups and medical conditions within your professional scope.
- Monitor patient referrals and laboratory results in accordance with diabetes care protocols.
- Facilitate diabetes education classes and personalized patient sessions.
- Deliver comprehensive diabetes health education while maintaining precise documentation.
- Collaborate with various healthcare teams to improve patient outcomes.
- Assist in the implementation of disease management models and adherence to clinical protocols.
- Input and track patient data for evaluation and reporting purposes.
- Engage in audits and corrective action initiatives as needed.
- Prepare and submit reports on activities and compliance.
- Follow established policies, guidelines, and professional standards.
- Participate in ongoing educational opportunities to enhance nursing competencies.
- Contribute to organizational performance improvement efforts.
- Carry out additional responsibilities as assigned.
Qualifications:
- Completion of an accredited nursing program.
- Possession of a valid LVN license in California.
- At least one year of clinical or medical-surgical nursing experience.
- Experience in Case Management is preferred.
- Bilingual in English and Spanish, with an understanding of Hispanic cultural nuances.
- Current certification in CPR.
