Job Summary

We are a Christian hospice organization dedicated to providing high-quality clinical care and the love and salvation of Jesus Christ. As a Registered Nurse Case Manager, you will play a vital role in planning, organizing, and directing hospice care with a focus on community health education and experience.

Key Responsibilities
  • Represent Empyrean Hospice with professionalism and compassion, ensuring the highest level of care for our patients and families.
  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop individualized care plans.
  • Coordinate patient care for assigned caseloads, working closely with the interdisciplinary care team to ensure seamless care delivery.
  • Counsel patients and families on meeting their nursing and related needs, providing health care instructions as necessary.
  • Assist patients with activities of daily living and facilitate their efforts towards self-sufficiency and optimal comfort care.
  • Document accurate and relevant clinical notes regarding patient conditions and care provided.
  • Communicate effectively with physicians and the hospice care team regarding patient needs and report changes in patient condition.
  • Provide a safe environment for patients, arranging equipment and services as needed.
  • Supervise ancillary personnel and delegate responsibilities as required.
  • Ensure adherence to standards of ethical business and clinical practice.
Requirements
  • Current licensure in the state and CPR certification.
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience in an institutional setting and 1-2 years of recent experience in home care.
  • Bachelor's degree, with one year of hospice care experience preferred.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Able to lift, position, and/or transfer patients, and able to lift supplies and equipment.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
  • Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.
