HIV Case Manager Job Description

Central Health is seeking a highly motivated and compassionate HIV Case Manager to join our team. As a key member of our Sexual Health Program, you will play a vital role in ending the HIV epidemic by providing intensive, client-centered services to individuals living with HIV/AIDS.

Key Responsibilities:

  • Determine eligibility and re-certification for housing case management, short-term rental/mortgage assistance, and utility aid services.
  • Serve as a subject matter expert on interventions impacting housing access and stability, providing time-limited housing assistance to prevent homelessness and promote stability.
  • Facilitate Ryan White Early Intervention Services (EIS) for newly diagnosed and returning patients.
  • Participate in weekly care conferencing with supervisor and peer Case Manager for HIV populations to ensure comprehensive management of housing and medical needs for mutual patients.
  • Refers ineligible households to appropriate supportive services, assesses their housing status and primary barriers, identifies social service needs, and coordinates warm hand-offs to the Case Manager specializing in HIV populations for patients with chronic, long-term needs.
  • Maintains client files and documents services using electronic health records and case management software.
  • Ensures compliance with federal and state guidelines, supports Service Plan development, and monitors general housing resources for case management clients.
  • Collaborates with eligible households to create budgets and explore strategies for increasing income or reducing expenses.
  • Works with Grants and Finance staff to ensure rental/mortgage and utility assistance payments are processed accurately and promptly.
  • Participates actively in the Sexual Health Program through meetings, client staffing, trainings, workshops, planning activities, special projects, and providing support/training to colleagues.
  • Collaborates with staff, informs clients about available services, and connects them with appropriate resources.
  • Establishes and maintains effective relationships with community resources, including those offering HIV testing, health, mental health, and social services to underserved populations.
  • Stays updated on HIV/AIDS, treatments, risk reduction techniques, chronic diseases, mental health, insurance, benefits, and case management through self-study and training.

Requirements:

  • Bachelor's degree in social work or similar field.
  • One year of case management experience required.
  • Valid driver's license.
  • Basic Life Support (BLS) certification.
