Care Navigator Role at Curative HR LLC


We are seeking an experienced Care Navigator to join our team in Austin, TX. The ideal candidate will have a passion for healthcare and excellent communication skills.


About the Job:


The Care Navigator plays a crucial role in helping patients navigate the healthcare system, engage in preventative care, and achieve their health goals. This is a full-time position that may require working evenings and/or weekend hours.


Key Responsibilities:


  • Ensure assigned members have great experiences seeking and receiving care.
  • Encourage members to engage more with their health by providing tools and educational courses.
  • Work with physicians and clinical staff to administer personalized, preventative care.
  • Optimize care and health outcomes for members.
  • Support operations of the practice by assisting with care coordination.
  • Improve patient experience by anticipating care interventions.
  • Educate patients on components of their health plan and answer questions.
  • Follow up with patients after health appointments to assist with follow-up steps.

Requirements:


  • 3+ years of experience in case management/care navigation/health plan/medical office role.
  • At least 1 year of experience working in a clinical setting.
  • Fluent in English (Spanish fluency a plus).
  • Outstanding written and verbal communication skills.
  • Analytical mindset.
  • Strong ability to use and learn new technology tools.
