Salary and Benefits

Job Description

As a Renewal Care Navigator, you will be responsible for supporting patients with complex kidney disease, ensuring they receive high-quality care and minimizing hospital readmissions. You will work closely with healthcare teams to coordinate patient care, identify social determinants of health, and implement effective interventions to improve outcomes.

Required Skills and Qualifications

To succeed in this role, you will need:

  • A current RN license and at least 2 years of experience in healthcare performance coaching
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients and healthcare teams
