Central Health is seeking a dedicated and compassionate Clinical RN Navigator to join our team in Austin, Texas. This hybrid role will be based in the Navigation Center, where you will play a vital part in navigating patients through complex healthcare environments for optimal care and outcomes.

The ideal candidate will possess excellent customer service skills, a high level of problem-solving abilities, and proficiency in Microsoft Office software tools. With a strong background in nursing and experience in an ambulatory clinic and/or hospital setting, you will collaborate with the patient's care team to optimize clinical outcomes. Your expertise will be utilized to review clinical progress, coordinate communications with referring physicians, and educate patients and family members on diagnosis, treatments, and medications.

In this role, you will also support transitions of care activities, enhance the patient experience by supporting provider teams, and participate in continuous quality improvement projects. As a key member of the healthcare team, you will maintain awareness of key performance indicators and coordinate interventions to prevent avoidable ER visits, hospital admissions, and readmissions.
