The salary for this position ranges from $79,000 to $87,000 per year, depending on experience.

Job Description

We are seeking a highly skilled Healthcare Navigation Specialist to join our team in Austin, Texas. As a key member of our navigation center, you will work collaboratively with patients, healthcare providers, and other stakeholders to optimize clinical outcomes and improve the patient experience.

Key Responsibilities:
  • Collaborate with the patient's care team to optimize clinical outcomes.
  • Review clinical progress and coordinate communications with referring physicians.
  • Coordinate scheduling of appointments as needed.
  • Assist with the referral process, tracking outcomes of referrals and treatments.
  • Educate patients and family members on diagnosis, treatments, procedures, and medications.
Required Skills and Qualifications:
  • Bachelor of Science in Nursing (BSN) preferred.
  • Two years of nursing experience in an ambulatory clinic and/or hospital setting.
  • Case Management or Navigation experience preferred.
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
  • Basic Life Support (BLS) certification obtained through approved American Heart Association or American Red Cross.
