About the Role

We are seeking a skilled Registered Nurse to join our team as a Healthcare Navigator - Homeless Services Specialist. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive care to marginalized individuals with complex health needs.

Job Summary

The successful candidate will be responsible for delivering high-quality nursing care to patients in our Respite and Bridge Programs. This hybrid position requires strong clinical skills, excellent communication abilities, and a commitment to building relationships with community partners.

Key Responsibilities
  • Provides nursing support for the service delivery of Respite, Bridge, and Mobile Healthcare Services.
  • Collaborates with the patient's care team to optimize clinical outcomes.
  • Oversees assigned team members, including but not limited to, assisting with establishing work assignments and overseeing flow of unit along with identifying staffing needs.
  • Assesses the physical, social, and emotional needs of patients and families in clinics.
  • Serves as an advocate, providing direction and support to the patient and family. Educates patient and family members on diagnosis, treatments, procedures, and medications.
  • Participates in the identification of patient needs for referral to resources that facilitate the continuity of care.
  • Enhances the patient experience by supporting Provider teams with in-basket management, refilling medications, and triaging patients in a timely manner following established protocols.
  • Performs clinical duties as assigned and competency validated in accordance with Standing Delegation Orders.
Requirements
  • Graduation from an accredited school of nursing - Associate's Degree
  • Two (2) years of nursing experience in a clinic, hospital or related setting
  • Unrestricted Registered Nurse (RN) License in the State of Texas
  • Basic Life Support - obtained through approved American Heart Association
Estimated Salary Range:

$65,000 - $85,000 per year, depending on experience.

This estimate is based on industry standards and takes into account the level of experience required for this role, as well as the cost of living in the greater Austin/Travis County area.
