Hospital Liaison Job Description

We are seeking an experienced Hospital Liaison to join our team at CommuniCare Health Services Corporate. As a key member of our sales team, you will be responsible for building and maintaining relationships with hospitals and healthcare providers to drive referral growth and admissions.

Key Responsibilities:
  • Develop and execute sales strategies to increase referrals and admissions from hospitals and healthcare providers.
  • Build and maintain strong relationships with physicians, discharge planners, and other hospital staff to ensure a steady flow of referrals.
  • Perform pre-admission assessments of patient referrals and evaluate patient needs to ensure a smooth transition to our care facilities.
  • Act as an educational resource for patients, families, hospitals, and healthcare professionals on our care services and facilities.
  • Monitor marketing activity and track referrals through CRM software to optimize sales efforts.
Requirements:
  • 2+ years of experience in sales or marketing to hospital accounts in the long-term care industry.
  • RN, LPN, LSW, or other medical credentials preferred, but not required.
  • Established referral contacts and community relationships.
  • Excellent written and verbal communication skills.
  • Ability to work independently and as part of a team to achieve sales goals.
