JOB SUMMARY

  • The Social Services Coordinator is tasked with executing designated social work functions and responsibilities within the organization. This role involves assisting the Social Services Director in planning, developing, and organizing the Social Services Department in alignment with existing policies and procedures, as well as Federal, State, and local standards and regulations, to effectively address individual resident needs while supporting overall facility objectives.
QUALIFICATIONS
  • Preferred Bachelor's Degree in a Human Services discipline (e.g., Social Work, Psychology, Counseling, Sociology, or a related field); a High School Diploma is mandatory.
  • Minimum of two years of experience in the social service or case management sector within healthcare.
  • Current understanding of Federal and State Long-Term Care Social Service regulations and the ability to interpret them.
  • Demonstrated skills in understanding and addressing the needs of residents, families, staff, and community agency personnel.
  • Proficient in reading, writing, and speaking English clearly. Capable of producing concise, meaningful clinical reports, assessments, documentation, and care planning initiatives while articulating the social and psychological needs of residents.
  • Ability to make independent decisions that prioritize the well-being of residents, staff, and families, while interacting professionally with residents, family members, visitors, and government agencies.
  • Exhibit leadership qualities under the guidance of the Social Services Director and collaborate effectively with other personnel.
  • Willingness to accept and utilize professional supervision, consultation, and training opportunities.
  • Experience in handling confidential information with discretion.
  • Marketing skills to effectively communicate the organization's Social Services Program to the community and other health professionals.
  • A genuine desire to work with diverse populations requiring long-term or short-term care.
  • Knowledge and ability to apply that knowledge in effectively addressing the needs of this population.
  • Ability to guide staff in therapeutic approaches for residents with dementia, behavioral issues, and mental health challenges.
  • Possess qualities of empathy, compassion, understanding, enthusiasm, and humor to engage effectively with residents, families, and staff.
  • Flexibility in working hours to ensure quality care.
  • Ability to manage emotional needs of oneself and staff.
PHYSICAL REQUIREMENTS
  • Ability to move intermittently throughout the workday.
  • Possess sight and hearing senses or use prosthetics that enable these senses to function adequately.
  • Function independently with flexibility, integrity, and effectiveness in working with residents, families, personnel, and support agencies.
  • Maintain good general health and demonstrate emotional stability.
  • Ability to cope with the mental and emotional stress associated with the position.
  • Ability to relate to and work with individuals who are physically or mentally ill, disabled, elderly, or emotionally distressed.
  • Free from physical limitations to participate in physical crisis prevention techniques as trained.
  • May be required to lift equipment and supplies and occasionally rearrange furniture.
  • Subject to exposure to infectious waste and diseases.
JOB DUTIES AND RESPONSIBILITIES
  • Welcome new residents, providing support and establishing a trusting therapeutic relationship.
  • Assess and document residents' feelings regarding long-term care placement and any changes in their emotional, mental, or physical condition.
  • Gain a comprehensive understanding of each resident's background, cultural history, and medical care needs to develop appropriate person-centered social service plans.
  • Document interactions with residents and families in assessments and care plans to advocate for residents as required by state standards.
  • Collaborate with the Interdisciplinary Team to prepare care plans based on comprehensive assessments for each resident.
  • Participate in discharge planning, ensuring consultation with other disciplines, families, and residents.
  • Conduct supportive visits to residents, documenting progress towards social service goals.
  • Provide active listening and supportive counseling to residents, facilitating their participation in programs that enhance their quality of life.
  • Act as an advocate for residents, liaising between them, their families, and community agencies.
  • Maintain an organized record system for social service information and contribute to the facility's Quality Improvement Programs.
  • Participate in safety committees as assigned.
  • Perform additional related duties as necessary.
SOCIAL SERVICE GOALS
  • Establish and monitor the achievement of facility and program goals through a collaborative approach.
  • Develop and maintain a therapeutic, person-centered social service program that enhances residents' quality of life.
  • Ensure compliance with all Social Services Policies and Procedures as well as State and Federal regulations.
ASSESSMENTS/DOCUMENTATION
  • Conduct and complete initial and ongoing assessments and care planning initiatives related to social service needs.
  • Document initial assessments and comprehensive social service evaluations within required timeframes.
  • Address and document behavioral incidents as necessary.
  • Complete all required documentation accurately.
SOCIAL SERVICE EDUCATION
  • Engage in continual education to maintain licensure and participate in educational opportunities.
  • Conduct staff training sessions as required.
  • Provide educational opportunities for families, residents, and staff.
  • Model appropriate interactions and approaches in daily activities.
DAILY LIFE OF SOCIAL SERVICE PROGRAM
  • Ensure a clean and safe environment for residents and staff.
  • Encourage family and community involvement to enrich residents' lives.
  • Supervise assigned staff to ensure continuity of care.
  • Market the Social Service Program effectively within the community.
  • Build and maintain relationships with local senior services and healthcare organizations.
  • Become a knowledgeable leader in the social service field.
OTHER
  • Perform any other necessary duties that support the Social Service Program and the overall needs of the facility.
