JOB SUMMARY
- The Social Services Coordinator is tasked with executing designated social work responsibilities within the organization. This role involves assisting the Social Services Director in planning, developing, and organizing the Social Services Department in alignment with current policies and regulations at The Alden Network.
- Preferred Bachelor's Degree in Human Services (e.g., Social Work, Psychology, Counseling, Sociology, or related field); High School Diploma required.
- Minimum of two years of experience in social services or case management within the healthcare sector.
- Current understanding of Federal and State Long-Term Care regulations and the ability to interpret them.
- Demonstrated skills in understanding and addressing the needs of residents, families, and community agency personnel.
- Proficient in reading, writing, and speaking English clearly. Capable of producing concise clinical reports and care plans.
- Ability to make independent decisions in the best interest of residents and staff.
- Strong leadership qualities and a collaborative spirit to work effectively with colleagues.
- Willingness to accept professional supervision and engage in ongoing training opportunities.
- Experience in marketing social services programs to the community and other professionals.
- Genuine desire to work with diverse populations requiring various levels of care.
- Ability to guide staff in therapeutic approaches for residents with mental health challenges.
- Possess empathy, compassion, and a sense of humor to foster positive interactions.
- Flexibility in working hours to ensure quality care.

- Assist in creating discharge plans that involve family and community consultations.
- Conduct supportive visits and maintain progress notes for residents.
- Provide counseling and facilitate resident participation in programs.
- Act as an advocate for residents, liaising with families and community agencies.
- Maintain organized records of social service information and participate in quality improvement initiatives.
- Engage in safety committee activities as assigned.
- Perform additional duties as required.
- Establish and monitor facility and program goals collaboratively.
- Develop a therapeutic, person-centered social service program to enhance residents' quality of life.
- Adhere to all relevant policies and regulations.
- Conduct initial and ongoing assessments of residents' social service needs.
- Document assessments and care plans within specified timeframes.
- Address and document behavioral incidents as necessary.
- Complete all required documentation accurately.
- Participate in ongoing education to maintain licensure and enhance skills.
- Conduct staff training sessions regularly.
- Provide educational opportunities for families and residents.
- Model appropriate interactions and approaches in daily activities.
- Ensure a clean and safe environment for residents and staff.
- Encourage family and community involvement in resident activities.
- Supervise assigned staff to ensure continuity of care.
- Promote the Social Services Program within the community.
