Position Overview

This role involves providing comprehensive clinical social work services within a primary care or home health environment. The successful candidate will offer support and guidance to the care team in managing patient needs. Responsibilities include evaluating psychosocial strengths and challenges to help patients achieve optimal independence. Collaboration with other professionals to develop a care plan is essential, along with providing short-term counseling and connecting patients with additional resources.



Key Responsibilities
  • Deliver effective social work interventions for patients of all ages and their families, including psychosocial assessments, advocacy, referrals, and case management.
  • Engage in consultation and coordination with a multidisciplinary team, participating in family and care conferences as needed.
  • Assess patient and family requirements for care, ensuring effective communication with healthcare providers. Collect and analyze relevant data throughout the care process.
  • Continuously evaluate patient and family needs, adjusting care plans based on responses and observations.
QualificationsExperience:
  • At least one year of prior social work experience is required.
  • Current KP Coalition employees may have this experience requirement waived.
Education:
  • Master's degree in Social Work (MSW) is mandatory.
Licensure:
  • Must hold one of the following licenses in Washington: Licensed Marriage and Family Therapist, Licensed Advanced Social Worker, Licensed Social Worker Associate-Independent Clinical, Licensed Independent Clinical Social Worker, or Mental Health Counselor.
Additional Skills
  • Exceptional written and verbal communication abilities.
  • Proven success in a collaborative, multidisciplinary team environment with a commitment to diversity.
Preferred Qualifications
  • Two years of post-master's experience.
  • Experience in care coordination within managed care settings, including inpatient and outpatient environments.
  • Knowledge of social case management, conflict resolution, and team-building practices.
  • Proficiency in Microsoft Office and familiarity with electronic health records.
