This role involves delivering a comprehensive range of clinical social work services within a primary care or home health environment. The successful candidate will offer support and guidance to the care team in managing patient needs. Key responsibilities include assessing psychosocial strengths and challenges to help patients achieve maximum independence and collaborating with other professionals to create effective care plans.
Key Responsibilities:
- Deliver tailored social work interventions to patients of all ages and their families, including psychosocial assessments, advocacy, referrals, and long-term care planning.
- Engage in consultation and coordination with a multidisciplinary team of healthcare professionals, participating in family and care conferences.
- Assess patient and family needs, ensuring effective communication with healthcare providers and ongoing evaluation throughout the care process.
- Adjust care plans based on continuous assessment and patient responses.
- Minimum of one (1) year of prior social work experience.
- Current KP Coalition employees may have this experience requirement waived.
- Master's degree in social work (MSW).
- Must hold one of the following licenses in Washington: Licensed Marriage and Family Therapist, Licensed Advanced Social Worker, Licensed Social Worker Associate-Independent Clinical, Licensed Independent Clinical Social Worker, or Mental Health Counselor.
- Strong written and verbal communication abilities.
- Proven experience working within a multidisciplinary team and a commitment to diversity.
- Two (2) years of post-master's experience.
- Experience in care coordination within managed care systems or long-term care facilities.
- Knowledge of social case management and conflict resolution.
- Familiarity with Microsoft Office and electronic charting systems.
